It came off because you didn't close down the handle didn't it. Done it myself - distracted while hitching up - drove off and it came off leaving a scar in the road which is still there to remind me outside our house.
The cable must be replaced to comply with regs and avoid possible insurance issues in the event of an incident.
I have also driven away from caravan without undoing the cable!
