My MH Horn works fine when engine is off but when I switch engine on it won't work.
Any ideas why?

If it's an older vehicle then probably as simple as poor connection or faulty horn which will be under bumper somewhere.

Usually, one side of the horn is always live and the horn button connects the other side to ground. So get a test meter and check it out.
