It didn’t cost me anything years ago, I made an appointment & took the paperwork round to them & they signed it - but I didn’t go to a doctor. The GP (or whoever) has to have known you personally for 2+ years, I think, and anyway our health practice has a notice saying they do not do references - they don’t have the time.
You should have access to other suitable people too - this passport example will apply for other cases which need someone to vouch for you:

PS - seems a bit onerous for a job application. I’ve never needed a counter signatory reference for any job applications: the time came from extracting the written reference from my current line manager when I was moving on.
Hi all thank you for replying. The job is for the Ministry of Justice.Actually construction cleaning a prison to be exact. Thats why i needed a Counter referee.Fiona W, exactly iv worried my self stupid. I cannot ask relatives and it would have been done by now as i have a lot of them in a trusted position.
The form is online and the Docs want it in paper form for maybe 30 days, i dont think they quite know how this system works . Dentist was willing but the deadline is today and she is not back in work until tomorrow.
On a positive note i had a call from an agency for the same roll and a lot nearer home.
