Disconnect the 12 volt connector at back of fridge and check voltage with engine running as needs at least 13 volts for the fridge relay to cut in for the 12 volt element to work. I had the same problem when engine running and voltage was OK,but the trouble was the connector was not making a good connection, therefore voltage drop. Sprayed connector pins with WD 40 and clipped connector back together. Then put a cable tie around it to keep it well pulled together and then black tapped around it to keep any moisture out. Been fine ever since. The connector can get damp air through the bottom vent due to location and connector pins oxidise.
