I am visiting Italy for the first time for 90 days in spring. I understand the maximum car and caravan length is 12m. My combined is 12.1m. Do they enforce this rigorously or should I be ok

You have been misinformed - your outfit is legal for Italian roads. But whether you will thread a very long caravan on to a typical Italian campsite is another matter.
The 12m limit applies to the towing vehicle only. Like most other European countries, the maximum combined length of a vehicle towing a trailer is 18.75m.

We managed to squeeze our tent (5.5m x 3.5m) plus car (berlingo) onto several italian pitches and just had enough room to put up a table and a couple of chairs to eat outside. Two trips, 2014 and 2017. Subsequent trips rented mobile homes or air b'n'b.
