Does anyone have a claim form they could scan and email to me please. I have made 3 requests for a form from the CC over the last 3 weeks and am still waiting. I know there has been postal issues with the weather but this is starting to get annoying.

  Is there not one in with your policy documents?  I'm not currently at home but I'm sure they send one out with the other paper-work.
