Switch it on a little while before you need it, switch it off when done. On ours, we can adjust the temperature when it's on gas - I have it so it's just too hot for washing up straight out of the tap (as it cools as a little when you are filling the bowl, as cold water is drawn into the tank) - no point in wasting gas heating the water to a temperature you can't use.
Safety-wise - it's vented outside and won't work if the vent is blocked in anyway (like when you forget to take the cover off as you are so used to using it on mains when it doesn't matter - guess who has done that!!!), so it's safe to leave on unattended, just a bit of a waste of gas. I've no idea how much it uses, I'm afraid!
